Caring Calls

Caring Calls

Caring Calls are routinely scheduled between home visits to assess your status and promote self -care and independence. Those on the Connect Telemonitoring program will be contacted daily for the first 14 days of care. Our Care Center team can also contact relatives and loved ones, as you tell us, to update them on your status, to give them peace of mind.

Telehealth is very important in reducing and/or preventing a return to the ER or hospital after hospitalization or illness/injury. The Connect telehealth monitoring program through Care Partners @ Home, uses our company’s Care Center clinicians to monitor patients who are on the program. A telehealth device is placed in your home and the Care Center team monitors the returns of the devices, makes an assessment of symptoms, identifies changes in your condition and coordinates follow up visits, as needed.

We are available 24 Hours a day, 7 Days a week.
